Liposuction, commonly known as fat removal, is a well-established procedure in plastic and aesthetic surgery. Unlike conventional cosmetic liposuction, this type of liposuction involves a circumferential and thorough removal of diseased fat cells. As a result, many of the burdensome symptoms of lipedema can be significantly reduced or even completely eliminated. Aesthetic outcomes also play an important role in treatment. Liposuction restores the original silhouette and gives back lost femininity: a well-proportioned body without lipedema. In many cases, accompanying skin tightening provides the necessary basis for this.

In contrast to aesthetic liposuction, which primarily aims to remove localized fat pads, lipedema liposuction focuses on alleviating disease-specific symptoms and complaints—while simultaneously aiming for a good to very good aesthetic outcome.

Reasons for liposuction

Lipedema is characterized not only by disproportionate fat distribution, which can affect the aesthetic appearance, but above all by complaints that can severely impair daily life.
Liposuction should be considered especially when lipedema symptoms intensify over time and quality of life is significantly restricted. The decision for the procedure is based on various factors—medical as well as functional:

When conservative therapies are not enough
Many patients experience only temporary or limited relief from their complaints through conventional treatments such as compression therapy, expanded physical activity, dietary changes, and lymphatic drainage.

High psychological distress
A lipedema condition can lead to considerable psychological strain, as those affected are often confronted with a changed body image and a lack of understanding from those around them. This often results in isolation that increases emotional imbalance.

Prevention of secondary conditions
Without tailored treatment, lipedema can lead to further health complications such as lipo-lymphedema (swollen, edematous backs of hands or feet) and knee and hip osteoarthritis due to uneven joint loading.

Which liposuction methods exist?

Liposuction has evolved over the years and is now considered a very safe procedure. Several methods are now established, including:

Power-assisted liposuction (PAL)

In our clinics, we perform liposuction for lipedema using the PAL method (Power-Assisted Liposuction, also called vibration liposuction). A defined amount of tumescent solution—among other components, adrenaline and a local anesthetic—is injected into the fatty tissue. This facilitates removal of fat cells while simultaneously minimizing the risk of bleeding. The diseased fat is then removed using a vibration-assisted suction cannula. This method is considered particularly effective and tissue-sparing. It can be used in all diseased body regions.

Ultrasound-assisted liposuction (UAL)

The UAL method uses ultrasound waves to liquefy fat cells before they are suctioned. A small ultrasound probe is introduced into the tissue through tiny incisions.

Laser-assisted liposuction (LAL)

The LAL method employs laser energy to liquefy fat cells. This technique not only aids fat removal but also promotes skin tightening in the treated area.

Water-jet–assisted liposuction (WAL)

The WAL method (water-jet–assisted liposuction) uses a fan-shaped, pulsed water jet to loosen fatty tissue from the connective tissue while simultaneously suctioning. The PAL method, by contrast, works with mechanical oscillations of the cannula, which can enable more efficient fat removal in hardened tissue—as is often the case with lipedema. Especially in scarred fat tissue due to prior liposuctions, the PAL method allows release from scar tissue and is the superior technique for that purpose.

Our decision to use general anesthesia for liposuction in lipedema is based on ensuring maximum efficiency and safety during the procedure. Under general anesthesia, we can remove significantly larger amounts of fatty tissue than would be possible under local anesthesia in an outpatient setting.

This approach allows larger treatment areas to be addressed in a single session, sparing patients multiple separate procedures. In one operation, we can suction about 10 percent of body weight in volume.

What influences the success of liposuction?

Patient-related factors

  • Body weight and fat distribution: With higher body weight and pronounced fatty tissue, more fat can usually be suctioned than in slimmer patients.
  • Skin quality & scarring: Good skin elasticity and minimal scarring make removal of diseased fat easier—this must be assessed individually.
  • Pre-existing conditions: Chronic conditions, Factor V Leiden, or metabolic disorders require careful evaluation of potential risks in advance.

Procedure-related factors

  • Choice of technique: With the PAL method, larger amounts can often be removed than with WAL.

Liposuction consultation and 3D scan for surgical planning

In our specialist centers, we use state-of-the-art technologies to improve planning and execution of treatments. A central tool is our 3D scan MyLIPOAvatar, which enables comprehensive body analysis. With this system, we measure patients’ body fat percentage very precisely before and after surgery. By creating a 3D avatar of each patient, we obtain accurate information about the current condition, allowing precise surgical planning.

The application is very straightforward: within 30 seconds, we create an exact image of your body on a turntable. This “virtual twin” not only allows measurement of body circumference but also detailed analysis of posture and determination of body fat percentage. This information is very valuable for our specialists and simultaneously provides you with a clear, transparent depiction of treatment progress and results.

The lipedema surgery

On the day of surgery, you first move into the prepared room. The hospital stay includes an overnight stay. You complete the necessary forms and choose your meals. The surgeon visits you and reviews the exact procedure again. The areas to be treated are precisely marked on the skin. This helps guide the liposuction accurately. You are then picked up by our nursing staff and taken to the OR. Efficient planning avoids long waiting times, helping to reduce additional anxiety.

After induction of anesthesia, very small, strategically placed incisions are made that will be barely visible later. We then inject the tumescent solution into both the deep and superficial fatty tissue under the skin. This solution causes the fat tissue to swell and separate from surrounding tissue, allowing gentler and more efficient suction.

Using a special vibration cannula, the swollen fat is then gently suctioned. To support healing and optimize the contouring result, tight-fitting compression garments are applied before you wake up from anesthesia. This compression is crucial to minimize swelling and promote contouring of the treated areas. The entire procedure is designed to be both effective and as minimally invasive as possible. If skin tightening after liposuction is required, it can be performed within the same procedure—for example using the Renuvion® technique, suitable for patients with moderate skin laxity and increased volume. Immediately after liposuction and on the following day, our LIPOPHYSIO® performs lymphatic drainage. This medical massage stimulates transport of lymph fluids and has a decongestive effect.


Liposuction aftercare

After liposuction, compression therapy plays a key role in the success of the procedure. It is important to wear custom-made compression garments that not only support healing but also significantly contribute to shaping the treated areas. We recommend taking a few days of rest after the operation to give your body time to adapt to the changes and new freedom of movement. To make the first days at home easier, you will receive a Take-Care bag from us. This contains essential components for initial care such as anti-thrombosis injections, prescriptions for medications (provided during pre-op counseling), supplements for wound healing, and a sick note. You will also receive emergency contact details for our doctors and a brochure with many tips for the time after surgery. Our exercise videos help you quickly regain mobility.

Risks and limitations of liposuction

We strive to minimize the risks of liposuction as much as possible. Despite the high safety of the procedure, there are also risks and limitations that patients must be informed about before treatment.

Individual risk factors include, among others, an elevated BMI. In such cases, liposuction requires particular expertise. Patient safety—even with a higher body mass index (BMI)—is one of our primary concerns in liposuction. Thanks to our modern and safe surgical techniques, we can successfully treat patients with a higher BMI as well. How painful is liposuction?

Everyone has a different perception of pain, but the majority of our patients describe postoperative pain as tolerable (often described as severe muscle soreness), especially compared to their prior lipedema complaints. It can also be well controlled with prescribed pain medication.
